Step 2. If Patient Having Individual Mediclaim Policy

Step 3. Book Appointment If You having Such Mediclaim Policy

Step 4. When You reached in hospital,then Consult yourself from the opthlamologist.

After that patient are send to TPA Department

Step 5. If you have any queries regarding your policy ask to the officer and know your policy details and package details.

Step 6. After Clearing Your Queries You need to fill the Preauth.Form Of your TPA Along with required documents.

Step 7. If Patient Having History of Diabeties And Hyper tension Then 1st do pathology investigation and take a MD Physician Fitness Report For the Surgery which advised.

Step 8. After that confirm your operation date with Hospital TPA Department.then Your Preauth. Form will be Send.

Step 9. After All the above procedure,when hospital received an provisional approval from your TPA, then single acknoledgement will send from hospital to patient.

Step 10. Put Antibiotic drop Before 2 days of your date of operation it is mandatory.
